Emergency Hazardous Material Release
The Police Dispatcher will ask a caller reporting an "Emergency Release" the following:
1. Location of the release?
2. Are there any injuries, illnesses or ANY negative symptioms (headache, dizziness, nausea, etc.) which resulted from the release? If yes, please describe.
3. What is the approximate amount of material that was released?
4. Is the release due to a leak? Yes _____ No _____
If yes, does it continue to flow? Yes _____ No _____
If it does continue to flow, answer the following questions:
At what "rate" is the material flowing? (e.g. "dribbling", "streaming", "gushing")
What is the approximate size of the container from which the material is leaking? (i.e. How much liquid does it hold? What are its dimensions?)
5. What is the identity of the material relesed?
What is the name of the manufacturer or distributor of the material?
What is the city and state of the manufacturer or distributor?
6. What hazards are known to be associated with the material? (i.e. Is it flammable, explosive, toxic upon inhalation or skin contact? Is it reactive, etc.)
7. If the material IS flammable, do you know any open flames or other significant ignition sources in the area of the release? If yes, Please describe.
8. If the identity of the material released is unknown, please describe it. (i.e. What are the physical characteristics of the material such as color, odor, etc. Is it a liquid/solid/gas?)
9. Has liquid been "contained" to prevent its spread? Yes _____ No _____
How was that accomplished?
10. Did the substance enter a drain and/or sewer? Yes _____ No _____
If yes, location?
